Our team uses all the resources in our capacity to overcome cancer health disparities and ensure that cancer patients and their families receive help with their struggle, be it related to healthcare or non-health related gaps in resources(social determinants, life food insecurity, transportation, utility issues, shelter and other issues).Too many people in our communities avoid getting any medical treatment because they either don’t have medical insurance, can’t afford expensive medical bills or don’t have an access to resources. To help these patients, we use NOLA-SDoH forms. All patients are required to fill out an
SDoH Social Determinants of Health (SDoH) are defined as “the conditions in which people are born, grow, work, live, age and the wider set of forces and systems that shape the conditions of daily life”. They are responsible for most health inequalities as they affect factors at a local, national, and global level. We believe that understanding and addressing SDoH is a pressing issue for the entire ecosystem involved in the delivery of healthcare (including diagnostic companies involved in precision medicine, pharma manufacturers, CROs, policy makers, physicians, and medical professionals in the communities they serve). A health care delivery model that factors in the correlation between SDoH and the following factors will enable us to identify the role and influence of all individuals to figure out a path that will promote equity, equality, and justice for all.

For patients who can’t afford medical treatment, NOLA (in partnership with CBCCA)will identify and locate resources for their entire course of treatment and medications, ensuring that financial constraints do not interrupt treatment. NOLA also provides knowledge to patients on the importance of timely cancer screening, emphasizing the impact it has on patients who avoid getting it done and end up diagnosed with advanced cancer. With the help of CCORN, NOLA also ensures that patients not only have access to medical screening but also have access to potential clinical trials alongside the latest medication and therapies. For patients who lack medical insurances, NOLA, with the help of its partners, can assist eligible patients in finding coverage. NOLA also ensures that its patients have comfortable environments to live in. With the help of

Conward Black has been a patient of Dr. Kashyap Patel at the Carolina Blood and Cancer Car Since 2004/5 for head and neck cancer and  again since 2014 for Essential Thrombocythemia.
